This rubber ball helps birthing women during labor. This woman is pregnant with her first son.

N:

Here there are approximately 600 births per month. During labor, the birthing mother is the center of attention. It is important to remember that the woman in labor must relax as much as possible. She must be able to choose the most comfortable position during contractions in order to facilitate birthing.

-Yae: Vem cá comigo.
-Yae: Come with me.

N:
As contrações ajudam a dilatação.

Andrea is 14-years old, and she is going to have her first baby. She has started to feel contractions. Contractions help to stretch the birth canal.

Certo?
Médico: Baixa a sua mão.
Enf. Isolda: Isso, isso. Vem, vem. Tá. Saindo. Vamos, isso, muito bem.
Médico: Parabéns!
Isolda: Naceu!

Yae (obstetric nurse): What you are doing right now is good for relaxation.
Nurse 1: Until pain fades away. When pain goes away, you breathe as usual, OK?
Andrea: Which of you two will inform my father?
Yae: When the contraction goes away, inhale normally, OK?
Doctor: Lower your hand.
Isolda: That’s it! That’s it! It’s coming! It’s coming. Good. It’s coming out. Let’s go. That’s it! Well done!
Doctor: Congratulations!
Isolda: Your baby is born!

N:

Normally after the baby is born, he is placed on his mother’s belly.
In case of Andrea, her baby’s umbilical cord was short and it had to be cut before the baby could be placed on her belly.

Yae: Parabéms!
Médico: Parabéns,viu?
Isolda: É, eu vou levar.

Yae: Congratulations!
Doctor: Congratulations, OK?
Isolda: Yes, I will take him.

Casa de GestanteÅF
Home of Pregnant Women

Enf.
Enf. Ausila: Prematuro?

Nurse Ausila: So why are you here?
Pregnant woman 1: Premature delivery.
Ausila: Premature?

N:

Here is the Home for Pregnant Women in Cesar Cals Hospital, where they try to provide a home-like atmosphere for the pregnant women.

Ausila: É água na cabeça, né? Entrou água e aí houve o aumento da cabeça.

Ausila: It was because of water in the head, right? Water entered and enlarged the head.

N:

Pregrant women at high risk and those who live far away from the hospital, or whose babies have some type of medical problem are interned until the day of birthing.

A gente ficava no quarto sozinho e dificilmente a enfermeira vinha.

Pregnant woman 2: In the hospital where I was, the room was ugly. We were left alone in the room and the nurse hardly showed up. It was so difficult! We were there, dying of pain and no one came. However, it is different here, because in the birthing room, the doctor is sitting and the nurse is going back and forth all the time. They never fail to show up.

Ausila: Não falta, né? Assistência é importante, né?
Mãe 3: É sim.

Ausila: Never fail to show up, right? Assistance is important, isn’t it?
Pregnant woman 3: Exactly.

E eu:-” Valha me deus!

Mother 4: I was kind of scared, you know. I was in the birthing room. I came in with a blood pressure problem. My blood pressure was very high and I was there for two days in the birthing room. However, then my pressure became normal and they wanted to bring me here. I waited for a vacancy in the infirmary, but none became available. Then the doctor arrived and said, “Let’s take her to the Home for Pregnant Woman”. I thought, “Oh my God! What’s that?” I was in that birthing room. In that room there was an ultrasound device, and a radio. I was frightened. I said, “Oh my God! Where are they going to take me now? I felt ill at ease and said, “Oh my God, to the private hospital? Then I have to pay...”

Instructor: Paper. We’re going to have to recut the paper so that it will be stiff enough to work.

Programa Kanguru
Kangaroo Program

N:

Premature babies are placed in the Kangaroo Program. It was given this name because the new-born babies are maintained in direct contact with their mothers. With this contact, the relationship between mother and child is strengthened and at the same time through smell, maternal milk production is stimulated.

Mãe: Sete meses. Naceu com 1295g, tá com1935g. De ontem para hoje ela engourdou 80g.

Mother: 7 months. She was born at 1295 g (2 lbs 14 oz), and now she weighs 1935 g (4 lbs 4 oz). Since yesterday she has gained 80g (2.8 oz).

Banco de Leite
Milk Bank

N:

Mother’s milk is the best and the most complete food, especially for the premature babies. In the Milk Bank, mother’s milk is collected by the mothers themselves and frozen. Nurses use the milk when necessary, calculating the correct quantity for each baby. The Milk Bank also benefits those babies whose mothers do not have sufficient milk.

N:

The Maternity Home was created by the State Secretary of Health, in September, 1998, in downtown São Paulo.

Mãe da gestante: Que vai nascer.
Mother of a pregnant woman: Don’t worry! The baby will soon be born.

Enf. Matilde: Terceiro? Foi uma cesária? E um parto normal? Por que foi cesária?
Nurse Matilde: This is your third baby? You had one Caesarean? And one normal birth. Why did you have a Caesarean?

Pregnant woman: The doctor said I didn’t have much space...
Matilde: Huh?

Mãe: -Não tinha espaço para a minha idade.
Matilde: -Como assim?
Pregnant woman: He said that I didn’t have space for a woman of my age.
Matilde: How could that be?

Pregnant woman: I didn’t have space for having a normal birth.
Matilde: The first one was normal?

Mãe: Cesário.
Enf. Matilde: Quantos kilos?
Pregnant woman: Ceasarean.
Matilde: How much did the baby weigh?

Mãe: Nasceu com 3.350 g.
Matilde: Foi há quanto tempo?
Pregnant woman: It weighed 3,350 g (4 lbs 6 oz) when he was born.
Matilde: It was how long ago?

Mãe: Foi em 94.
Matilde: Há quatro anos e meio?
Pregnant woman: It was in 1994.
Matilde: It was four and a half years ago?

Mãe: -É cinco.
Matilde: Vai fazer cinco anos?
Pregnant woman: Five.
Matilde: It’s going to be five years?

Matilde: E o parto normal, há quanto tempo? Há, peraí. Você teve um aborto e uma cesária? Jura? Nunca teve parto normal? Só aborto? Oh meu Deus! Você sabe que aqui a gente faz parto normal.

Matilde: And the normal birth, how long ago. Oh, wait. You had one abortion and one Ceasarean? Oh dear! You’ve never had a normal birth? Only an abortion? Oh my God! You know that here we do normal birth.

N:
Esta gestante procurou a casa do parto poque queria dar a luz naturalmente.

This pregnant woman sought for a Maternity Home because she wanted to give birth naturally.

Matilde: Massas, evitar comer muitas massas.

Matilde: Noodles, avoid eating a lot of noodles. Eat more vegetables, fruits. OK? Food like flour, beans.... you have to avoid. If you feel anything irregular, come back as soon as possible, but I think in your case it’s fine.

N:

Matilde is one of the obstetric nurses of the Maternity Home. Ever since its inauguration, she has reconciled her work at a big hospital with her responsibilities at the Maternity Home.

N:
In this case the patient did not develop normal labor and they were forced to take her to the hospital.

N:
The Maternity Home works closely with nearby hospitals. Depending on the condition of pregnant woman, she can be transferred to one of these hospitals. When this happens, the Home has an ambulance which is always available.

Matilde: We knew that she had a mioma from the beginning. It was necessary to give natural childbirth a try, but the contractions have essentially stopped, so I called the hospital today.

Matilde: -Como vai? Tudo bem?
Matilde: -Quantos kilos?
Mãe: Nasceu com 4210g.
Matilde: How are you? Everything well?
Matilde: How much did she weigh?
Mother: She weighed 4,210 g (9 lbs 4 oz).

N:
For Matilde, receiving a return visit from a mother with her baby, after they have left the Birthing Home, is the most gratifying time. The emotion of these visits is what motivates her to continue her work of such great responsibility in the community.

Hospital Santa Luiza de Marilac (Aracati)
Santa Luiza de Marilac Hospital (Aracati)

N:

Here in Santa Luiza de Marilac Hospital in Aracati, encounters of pregnant women and their companions were implemented so that they could receive training and exchange information during the pre-natal period.

Yae: So until three or four months of pregnancy, you often feel like vomiting. It’s because of hormonal changes.

Enf.

Ángela (nurse): You do not want to have an empty stomach. Because an empty stomach increases the formation of gastric juice and will increase the probability of vomiting. Do you understand? So you have to avoid this. Even if you just eat a small quantity, but you cannot leave your stomach empty.

Ángela: The cervix will gradually open more, OK? These joints should be exercised, because if they are not, you will have greater difficulty at the time of childbirth. So whenever you wash your clothes, move like this.

Mãe: É lambada.

Pregnant woman 1: It’s the lambada.

Ángela: Yes that’s exactly right!. You have to dance like lambada. Listen, it is just like I said, this problem of sexual abuse, this right not to have sexual relations...

Pregnant woman 1: ...Only if it were with some other person, with my husband, no!

As alteram, ne?

Ángela: Just like I said, this hormonal shift changes a lot of women’s disposition. It affects women, see? Some women become happier, others become more moody. Some women are really, that friend of yours whom you considered to be your friend, she started to abuse you because of a problem which occurred between them.

Pregnant woman 2: Many times the husband who used to like her stopped liking her and is ready to leave her. That can be changed to bring them even closer.

Pregnant woman 2: Then sometimes I talk to my husband. He says, “birthing is your business.” Then I say, “No, it’s our business.” Then I asked him if he could assist me during the childbirth. And he said “No, because it’s not a matter for men”. Sometimes it’s really...It’s good that husbands see how much their wives suffer in order to give them sons.

Pregnant woman 3: When the husband comes, the baby is neatly dressed in the cradle, so they never see us undergoing such pain.

Assistant nurse: Husbands should come to the meetings, to the consultations with the doctor and all. If not, they will never understand. If you insist on their coming only on the day of childbirth, it will be even more difficult to bring them.

Yae: When the husband arrives at the time of childbirth, he is shocked to see the bleeding and your screaming. You look so different from normal. It’s good if you prepare together from the outset until the birth. You have to have a companion. This has to be the husband’s responsibility.

N:

He is going to be a father for the first time and arrived at the hospital with his wife.

Pai: Estou ansioso para ver o meu filho.

Father: I cannot wait to see my son.

Gestante 1: Pronto? Tá passando a dor?
Pregnant woman 1: It’s done. Are you feeling much pain?
Pregnant woman 2: Not too much.
Pregnant woman 1: Don’t you want to sit down?
Pregnant woman 2: No. I want to stand so that the baby will descend faster.
Pregnant woman 1: If that’s the case, I will stand, too. I will accompany you.

N:

Natural childbirth results from uniting the energy of both the mother who wants to give birth and the baby who wants to be born. This is a uniquely gratifying opportunity for women, and it helps each woman to better understand her own body, emotions and feelings.

Joana is 18 years of age and she is going to experience her first childbirth. She said that her boyfriend abandoned her when he learned that she was pregnant. But it did not adversely affect her, because she didn’t like him too much anyway.

N:

Josilene is 19 years old. This is going to be her second childbirth, however, this is the first with her current husband.

Irmã Faustina: Tá bom? Tá bom? Olha a música maravilhosa para relaxar. Gosta de música?

Sister Faustina: OK? What beautiful music for relaxation! Do you like music?

Assistant nurse: Where did you go?
Father: I went out to eat lunch.
Assistant nurse: How brave you are! Eating lunch at this time!
Father: Yeah, it’s because if I ever faint, it’s better to faint with full stomach than with hunger.

Meu amor!

Assistant nurse: Breathe!
Sister Faustina: That’s right! When the baby turns around, don’t push too hard at once. Push gently. Take it easy. The baby will soon be born.
Faustina: Just a little bit more. Just a little bit. That’s it!
Faustina: Just a little bit more, don’t push too hard. Take is easy, OK?
Mother: O my love!

Irmã: Oh! Que beleza! Deixe eu ver o rostinho dele! Que fofinho! Que lindo! Vai chamar logo ao pai!, vai.!
Irmá: Você já olhou! É um menino! Obá!

Sister: Oh! How beautiful! Let me see his face. How soft he is! How handsome! Go get his father quickly! Go!
Sister: You’ve seen! It’s a boy! Wow!

Mãe: Parece que não quer mamar?
Irmã Faustina: Debe estar cansado. Da viagem.
Mãe: Viagem longa, né.
Irmã: Viagem longa.

Mother: It doesn’t seem like he wants to suck my breast.
Sister: He must be tired from the journey.
Mother: Long journey, isn’t it?
Sister: Long journey.

N:

Right after birth, the baby becomes awake for more or less 2 hours. In the first moment of life, the contact between mother and baby is very important, and it is going to influence a lot their relationship in future.

Assistente de enfermagem: Respira pelo nariz devagarinho.

Assistant nurse: Breathe slowly through your nose.

N:

The Joana’s cervix has not completely dilated yet. Contractions started at home and she has already been in the hospital for 7 hours.

Pai: Olhe, meu filho! Você vai ser um artista! Olhe para a câmera! Deste tamanho e já está sendo filmado! Ele vai ser um ator de novela.

Father: Look, my son! You are going to be an artist. Look at the camera! So small and you are already being filmed. He is going to be a soap opera star.

Avó 1: Sou a mãe dela.
Grandmother 1: I am her mother.

Pai: Ave María, Mãe! Pegue ele direito.
My Goodness, Mother! Hold him correctly.

Avó 2: Ave María! Que besteira!
Grandmother 2: My Goodness! What a mess!

Father: Mom seems to have the baby sideways!

Avó1: Gostei dessa boca! Parece um botãozinho!
Grandma 1: I like his mouth. It looks like a little button!

N:
Estas são as avós do bebê. Elas também querem segurar o recém-nascido.

These are the baby’s grandmothers. They also want to hold the newborn.

Irmã : Isso....Isso....
Sister: That’s right...That’s right!

Irmã : Segura e faz força para baixo. Faz força como se fosse fazer cocô.
Sister: Hold this and bear down. Push as if you’re having a bowel movement.
Irmã: Aí! Olha a cabecinha! Isso! Opa! Calma aí.
Sister: There! Look at his little head! That’s right! Oh! Calm down!

Enfermeira: Mateus! Pronto! Lindo!
Nurse: Matthew! Ready! Beautiful!

Joana said that if she had a boy, he would be called Matthew.

Irmã: Olha! Que maravilha!
Sister: Look! How wonderful!

N:
The person who cut Matthew’s umbilical cord was Joana’s mother.

Joana: Nem acredito que é macho!
Joana: I cannot believe that he is a boy!

Irmã: Queria mesmo?
Sister: Did you want to have a boy?

Joana: Era, queria menino! Tou satisfeita.
Joan: Yes, I wanted a boy! I didn’t want a girl, but I would have been happy with whatever God sent. But God sent a boy, so it is even better. Boys are less work! With a girl, you have to spend more money! A boy, we don’t. If we dress him in just underpants it’s fine! I am happy.

Sister: This is a placenta. This is what connected you and the baby. Now you are separated. However, you are tied in another way now. You are tied with love!

Grandmother: They were already tied with love, but now they are even more tied.

Irmã Faustina:
Sister Faustina:
Humanized birthing should be assistance for what is going to happen... everything.

It’s important that the people who work in the maternity clinic, the birthing team, should be trained staff so that they will treat the client with great respect.

This respect should be shown principally in freedom of choice. The woman who is going to have a baby, is alreadyunder stress, and she must be respected in all regards.

This respect should be shown, not only at the time when she arrives at the clinic, but should be from the time when she leaves her house, when she gets into the car, and when she arrives at the maternity clinic.

However, in the moment she arrives at the clinic, this respect should be even greater, because the time has come that there will be two lives... the mother and the baby. And the health professionals should give her liberty of choice and security.

She should feel very secure in this circumstance. Principally at the moment of birth, freedom of choice to put her legs wherever she wants, to lie on her side, or to take whatever position she wants to take. What’s important is that she feels secure under these circumstance.
